We MUST already have your email address on file for you to receive your PIN number.
If you have a spouse or child which you want under the same email address they also must have that email address entered on their account. If your request for a PIN number does not work it probably means we do not have that email address associated with the patient you are trying to access.
If you are having difficulties please call patient relations at (909) 883-8611 to have them update your account.
It is your responsibility to keep your PIN number private. Anyone you give your PIN number to could access your patient portal account and view your medical record information.
